2. CONSENT AND RELEASE FORM Download

Please sign the “Consent & Release Form”. NOTE: This must be signed by your parent or legal guardian if you are under 18. Submit by email, fax or regular postal service

3. PHYSICIAN’S REPORTDownload

All participants must have a physical performed and signed by a licensed M.D. or licensed health care practitioner, including date of last tetanus booster, within 6 months of date of application. Submit by email, fax or regular postal service

4. LETTERS OF RECOMMENDATION/REFERENCE

For first-time athletes, please provide us with two letters of recommendation/reference . . . Submit by email, fax or regular postal service

5. MEDICAL INSURANCE

You will be notified by email &/or regular mail if your application is accepted. Do not make travel arrangements until you have received your notification. All applications must be postmarked by 12/22/16.  An acceptance email will be sent after 12/16/16.

Each participant is responsible for making his/her own transportation arrangements to/from Denver International Airport during the approved arrival & departure times as noted on the acceptance letter.  Upon acceptance you will need to inform us of your flight information by 1/9/17 to hold your space.  Any athlete that is put on the waiting list will be notified of any openings after 1/13/17.

All participants must be physically fit to participate in strenuous winter activities (i.e. cross-country skiing, snow-skiing/boarding, etc.) at high elevation, or summer activities (i.e. surfing, water-skiing, wake-boarding, etc.).  All participants should be between 14-30 years old.
